Position Title: Client Services Coordinator

Classification: Full-time, 40 hours per week, non-exempt, $20/hour

Primary Work Location:  Fremont, CA – SOS Meals on Wheels South Office

SOS Meals on Wheels is a 501-C (3) non-profit committed to providing home delivered meals and a daily check-in for vulnerable seniors in our community. SOS Meals on Wheels produces and delivers 2,500 meals daily for SOS senior clients living in Castro Valley, Hayward, Oakland San Leandro, San Lorenzo, Fremont, Newark, and Union City. As advocates for senior citizens, our mission is to provide a nutritious meal delivered by a friendly face that also acts as a wellness check, allowing seniors to remain independent in their homes for as long as safely possible. 

Job Overview: The Client Services Coordinator reports to the Program Manager and is involved with every step of the client intake and assessment process collaborating with the Client Services team members to comply with program requirements and ensure quality of care for SOS Meals on Wheels clients.

 Essential Responsibilities:

· Work with Program Manager and Client Services team to continue to develop and implement best practices for client intake and assessment procedures.

· Perform initial client screenings in a professional and positive manner, including all required data gathering and determining eligibility.

· Conduct scheduled assessments with clients to verify continued status for meals.

· Update the client database with any changes to client accounts.

· Call clients and their emergency contacts for well check when clients are not home for meal delivery.

· Prepare Welcome Packets, Route sheets and other administrative duties as needed.

· Stay current with local senior resources to make appropriate referrals for clients.

· Maintain a comprehensive and accurate Salesforce client database, including demographic information, dietary restrictions, delivery schedules, and other relevant details.

· Regularly update and verify client information to ensure data integrity and compliance with confidentiality standards.

· Participate in team meetings to ensure a coordinated and efficient workflow.

· Additional duties will be added/modified as the program continues to evolve.

Requirements:

· A commitment to the mission SOS Meals on Wheels and serving older adults.

· Excellent customer service skills while working with high need populations.

· Compassion and empathy for the elderly and individuals facing challenges.

· Good verbal and written communication skills.

· The ability to prioritize and balance conflicting demands.

· Excellent computer skills - Microsoft Office Suite required. Knowledge of Salesforce, or similar databases preferred.

· Effective team building and conflict resolution skills.

· Ability to work both independently and as an effective team member.

· Vehicle in good working condition, a valid California driver’s license and proof of auto insurance.

Working Environment:

· Most work is conducted in an office or field environment.

· Depending on need, may require frequent or occasional driving.
