Organization Mission and Summary: SOS Meals on Wheels is a 501(c)3 nonprofit committed to fostering the independence and dignity of older adults by reducing food insecurity and isolation, with daily nutritious meals, friendly visits and connection to support services. We produce and deliver over 500,000 meals annually for homebound seniors living in Oakland, Hayward, San Leandro, Castro Valley, San Lorenzo, Fremont, Newark and Union City. As advocates for senior citizens, we provide a nutritious meal delivered by a friendly face that also acts as a wellness check, allowing seniors to remain independent in their homes for as long as safely possible.
Position Summary: The Client Services Coordinator–Assessor conducts comprehensive in-home assessments to determine program eligibility and identify clients’ needs related to activities of daily living, mobility, medical conditions, nutrition risk, food security, social isolation, and overall safety and well-being. Based on assessment findings, the Assessor develops appropriate service and referral recommendations, follows up as needed to support connection to community resources, and follows all applicable organizational and legal reporting requirements related to suspected elder or dependent-adult abuse or neglect. The Assessor will play a crucial role in supporting our mission of providing nutritious meals and compassionate services to homebound seniors in our community, and to achieve the vision set by our Strategic Plan.
Accountability: Reporting to the Client Services Manager, the Assessor will also work closely with the Director of Programs and the rest of the Client Services team.
Essential Functions & Responsibilities
Assessment:
Schedule assessment appointments by phone and conduct in-home visits.
Use standardized tools to assess program eligibility, functional ability, medical conditions, nutrition risk, food security, and social isolation.
Assess clients’ need for services offered by SOS Meals on Wheels and make appropriate community referrals. (i.e.: Mercy brown bag, IHSS, Paratransit, APS, etc.).
Communicate with clients that no longer qualify for meal services or are non-compliant with the program.
Data Collection and Database Management:
Utilize Salesforce database to access assessment due reports.
Update client information and assessments in Salesforce database.
Program Support Development:
Assist with training and development of nursing student interns.
Attend occasional client outreach events.
Attend staff and team meetings.
Support and participate in program initiatives.
Requirements
Associates degree or a minimum of 2 years’ experience in business administration, community health and/or social services. Preference will be given to those whose experience includes working with seniors/disabled persons and/or within the nonprofit sector.
Excellent verbal and written communication skills
Ability to remain calm under pressure in a fast-paced environment
Demonstrated conflict-resolution and de-escalation skills, including the ability to communicate difficult decisions with empathy, clarity, and professionalism.
Ability to build trusting, professional relationships with clients and caregivers and conduct sensitive interviews while maintaining appropriate boundaries and respecting client dignity.
Demonstrated ability to engage diverse communities with cultural humility and respect.
Highly organized, deadline focused, and detail oriented with data and complex paperwork
Proficiency in Salesforce or other similar CRM database and Microsoft Office Suite (Teams, Outlook, Word and Excel)
Commitment to the SOS Meals on Wheels mission and serving older adults
Ability to be an individual contributor as well as be a team player
A reliable, working vehicle, valid driver’s license, as well as applicable insurance
Working Conditions
Work will be conducted in the homes of clients and in an office environment.
Work involves travel throughout central Alameda County from Oakland to Hayward using a personal vehicle. Mileage is reimbursed in accordance with SOS Meals on Wheels’ current mileage-reimbursement policy.
Work includes driving, walking, and climbing stairs, and may be performed seated or standing for prolonged periods of time.
Work involves regular use of a computer and common office equipment.
Some light lifting and bending required.
